This page contains all necessary forms for the SBMS Band. Items marked with an asterisk (*) are required to be read and filled out when a student registers for Band.
**SBMS Handbook 2022-2023*– reading of this document is required by both students and parents, and contains all guidelines and policies that dictate procedure for the South Belton Band. The last page must be signed by the student and parent/guardian and turned in to the band director.
*Instrument Check-out Form – this form is required for parents of students who play an instrument owned by SBMS. Parents can select their preferred payment plan of $100 for the school year that is non-refundable. Students will not be allowed to check out an instrument until this form is completed by the parent and turned in to the band director. Hard copies are available in the Band Hall.
*Private Lesson Form – this form is for parents to consider private lessons for their student.
Private Lesson Scholarship Application – this form is to apply for a lesson scholarship to help defer the cost of private lessons. Please turn in to your band director or email to sara.wiley@bisd.net.
*Uniform Shirt and Decal Order Form this form is required for students and parents to order their uniform shirt, spirit wear, and auto decals with the regular Band logo customizable with their student’s name and instrument.
Tuner Check-out Form – this form is for students to fill out who need to borrow a school-owned tuner for the school year. This must be signed by the parent and turned in to the band director.
All-Region Band Audition Form– this form is for students to fill out who are committing to audition for All-Region Band. This form is due no later than October 9, 2020.
Permission to Leave Band Event with Parent – this form is to be filled out when parents wish to pick up their student directly from a band event rather than ride the bus back.
After Performance Evaluation Form – this online form is for students to self-assess their recent band performance.
Music Listening Questions Form – this online form is for students to answer questions about the music they hear in class.
Rehearsal Guidelines Self-Assessment Form – this online form is for students to evaluate themselves on their efforts in following our rehearsal etiquette guidelines.
Music Request Form – this online form is for students who have lost or misplaced their music and need another copy… or maybe someone stole it!
